CottrillsCare App Privacy Policy

Overview

Cottrill’s Pharmacy, Inc.  (“we” or “us”) have adopted this policy (“Policy”) to describe the types of information we may collect or that you may provide when you download, install, access, or use the CottrillsCare App (“Application”), how we will use and disclose such information, whom we will share it with, and what measures we will take to ensure it remains private and secure.  In this Policy, when we refer to “you”, we mean only an authorized user of the Application to access information about your account(s) maintained with us.

You should read and understand this notice alongside the terms of services notice connected with Terms and Conditions of Use applicable to the Application. 

Information Collected

We collect information from and about you:

  • directly from you, when you provide it to us, and
  • automatically when you download or use the Application.

Specifically, we may ask you to provide or otherwise collect nonpublic personal information about you which may be used to identify you, including the following:

  • your name, prior names, age, and date of birth;
  • contact information, such as current and prior addresses, telephone numbers, and email addresses);
  • government-issued personal identifiers, such as a social security number or a taxpayer identification number); and

We may also collect information that is about you but does not identify you, including the following:

  • records and copies of your correspondence with us, if you contact us;
  • information you provide by filling in forms within the Application, including at the time of registration of the Application or requesting information or services of us via the Application; and
  • queries made and information accessed using the Application.

When we ask you to provide personal information, you are not required to comply with our request. However, failure to supply any of the information that we request may result in your ability to access and use the Application or any of its functions being limited.

Automatic Collection of Information

 When you download, access, and use the Application, we may automatically collect certain information, including as follows:

 

  • Usage Information. When you access and use the Application, we may collect details of your access to and use of the Application, including the resources that you access and use on or through the Application.
  • Device Information. We may collect information about the mobile or other device and Internet connection used to download, access, and use the Application, including a device’s unique device identifier, IP address, operating system, mobile network information, and device’s telephone number.
  • Stored Information. The Application may access metadata and other information associated with files stored on the device(s) you use to download, access, and use the Application.
  • Location Information. The Application may collect information about the location of your device when the Application is being used.

If you do not want us to collect any of the foregoing information, do not download the Application or, if already downloaded, delete the Application from your device. 

Collection of Information by Third Parties

When you use the Application or its content, certain third parties may use automatic information collection technology to collect information about you or the device(s) on which you download, access, and/or use the Application.  These third parties may include:

 

  • the application store or other platform through which you download the Application or any update thereto;
  • advertisers, ad networks, and ad servers;
  • analytics companies;
  • your mobile device manufacturer;
  • your mobile service provider;
  • your Internet service provider; and
  • other third parties.

 We do not control, and have no responsibility for, the collection of information by third parties or how information collected by such third parties is used, secured, maintained, or disclosed.

 

Use of Collected Information

We use information that we collect from or about you via the Application for the following purposes:

  • to provide you with the Application and its contents and information, products, and services accessible through the Application;
  • to evaluate and develop Application functionality and updates and enhancements to the Application;
  • to respond to questions and other inquiries you submit to us or carry out instructions you give to us;
  • to provide you with notices about your account(s) or products or services we offer or provide to you;
  • to perform our obligations and enforce our rights under any agreements between you and us, including the Application’s Terms and Conditions of Use and agreements unrelated to the Application; 
  • to notify you when Application updates are available and/or of changes to content within or to products or services we offer or provide through the Application;
  • for any purpose for which you voluntarily provide information;
  • to comply with our obligations under applicable law, subpoenas, and the orders of courts and other governmental authorities;
  • when we believe it is in the public interest to use your information, such as for the purpose of detecting or preventing a crime; and
  • for our other legitimate business purposes.

 

Disclosure of Nonpublic Personal Information

We may disclose aggregated or de-identified information about users of the Application without restriction.

We may disclose other information which we collect:

  • to our affiliates;
  • to contractors, service providers, and other third parties we use to support our business activities, including by providing marketing services, evaluating requests for products or services, performing general administrative activities, assisting in processing transactions, and providing other services;
  • to an actual or proposed buyer or other successor in interest to some or all of our assets or those of one or more of our affiliates in which information about the users of our Application is transferred;
  • to third parties involved in development, improvement, and/or distributing the Application;
  • to fulfill purposes for which you provide information;
  • for purposes disclosed to you by us when you provide us with information; 
  • when you consent to disclosure;
  • to comply with the requirements of applicable law, subpoenas, and the orders of courts and other governmental authorities;
  • to perform our obligations or exercise our rights under any contract between you and us, including the Application’s Terms and Conditions of Use and agreements unrelated to the Application; and
  • if we believe that disclosure is necessary to protect the rights, property, or safety of us or our affiliates, our customers, or others.

We do not disclose any nonpublic personal information about you to third parties, except as permitted or required by applicable law.

 

Information Security

We maintain physical, electronic, and procedural safeguards that are designed to protect personal information we collect from users of the Application from unauthorized access, use, alteration, and disclosure.

The transmission of information via the Internet and mobile platforms is not completely secure, so we cannot guarantee the security or integrity of your information which is transmitted through the Application.  We also have no responsibility for the circumvention by you or a third party of any privacy settings or security measures used to protect the integrity or security of your information within or provided through the Application.

It is important that you maintain the confidentiality and security of any username, password, or other identifier or information you may use to access or use the Application, as if someone gains access to the same, the security of your personal information within or accessed through the Application may be compromised.

 

Compliance with Applicable Law

We will collect, use, maintain, and disclose information in accordance with the requirements of applicable law.  The Application has been developed for use by users located within the United States of America and only in a manner consistent with the Terms and Conditions of Use.

Minors

The Application is not intended for use by persons under the age of 18, and we do not knowingly collect personal information from such persons.  If you believe that we might have collected any information from or about a person under the age of 18 without parental consent, please contact us at: (716) 681-1275.

 

Changes to this Policy

We reserve the right to supplement or otherwise modify this Policy at any time without notice to you and without your consent.  Your downloading or use of the Application after we modify this Policy constitutes your acceptance of this Policy, as it has been modified.  Therefore, we recommend that you review this Policy regularly for changes. The effective date of this Policy, as stated above, indicates the last date it was revised or materially changed.
